Referral & Assessment
Reach Out
Contact your family physician to get a referral for sleep testing at Maximum Sleep Care. please feel free to download our simple referral sheet to take to your primary care physician in order to receive a sleep-related assessment.
Have your family physician send the referral form to our clinic and then phone us to book an appointment. We will complete a series of thorough sleep assessments (including consults, interviews, laboratory tests, and home studies) and track your specific results.
We share the results of your sleep assessments with a certified pulmonologist or ABSM (American Board Certified Sleep Medicine) physician for their professional analysis.
Treatment
Together with your physician's directives, our team of specialists will customize an individualized treatment plan for your specific diagnosis and concerns.
We supply you with the necessary supplies and equipment for ongoing home care and treatment, such as a CPAP machine.
Diagnosis
The pulmonologist or ABSM Physician assigned to your case provides both you and us with an official diagnosis for any underlying sleep disorders or other health concerns.
We communicate with you and your primary care physician and pass on the results of your sleep assessments as well as clearly explain your diagnosis.
Support
We make ourselves available for ongoing support and modification of your personalized treatment plan.
We assist in processing your insurance claims and health care coverage through programs such as Alberta Blue Cross, or other employee benefit programs. Please note we provide direct billing for specific providers
